Job Title: Home Support Worker
Team: Reablement – Home Support
Hours: 28 per week
Grade: D SCP 6 £23,893 to SCP 11 £25,979 + 7% enhancement weekend working
(Pro rota salary based on 28hrs is £18,081)
Contract: 4 x Permanent and 2 x 6 Month Fixed Term

What do we do?
The service provides a stepping stone to independence, by supporting people to regain lost skills, learn new skills, and adapt to the challenges that living independently can present.
You will work within a CQC “Good” rated - Home Support Reablement Council Team, and will have an opportunity to work with other professionals such as our colleagues in the NHS, Social Workers, Occupational Therapists etc to provide all aspects of care, which includes but not exhaustive of:
• Moving and Handling
• Personal Care
• Administering medication
• Ensuring service users dignity and wishes are at the centre of the care provided.
Home Support Reablement is a short-term service, meaning there is a quick turn over of service users.

The role of a support worker is to support people in their own home who are at risk of needing social care or have an increased intensity of care to:
• regain functioning,
• maintain life skills,
• rebuild their confidence,
• promote wellbeing.

What will your working week look like?
• 28-hour contract
• Rolling rota
• Variety of early, lates and weekend shifts
• Working between 7am – 10pm
• Flexible approaches to shift patterns, including split shifts (see attached rota examples)

About You
Do you have…
• a Full Driving License and access to a vehicle to use for work purposes?
• a passion to support and care for adults in the community?
• the ability to work on your own initiative?
• the ability to communicate clearly with people of all levels?
• the skills to work in the community to encourage people to engage and reach their potential?
No experience is required for the role of home support worker, but a commitment to want to work in social care is essential. A comprehensive induction and training programme will be provided on succession into the role.
You must ensure that the service is delivered to the highest achievable professional standards to support vulnerable adults with both rehabilitative and / or mental health needs to retain community living.
Your work will be compliant with all CQC regulations, and Bolton Council policies and procedures.
